The items marked with an asterisk are required for the form to be submitted to our Service Managers.

Call 765-269-7283 for the following Priority (Emergency) Service Requests:

• No Electricity throughout the apartment
• No Heat in temperatures below 55°F
• No Air Conditioning in temperatures over 88°F
• No Water throughout the apartment
• Water flooding into the apartment (if possible please shut off the valve to stop the flooding)
• Smoke Alarms not working properly
• Situations where access to the apartment may be compromised (i.e. broken windows or broken locks)
